  • Birth Certificates

    Request a certified copy of a birth certificate issued by the State of Connecticut. You may request it either by mail, online via VitalChek. If you are requesting a birth certificate for someone other than yourself or your child, you will need to submit proof verifying your relationship to the person whose birth certificate you are requesting.

  • Marriage Licenses

    You and your spouse-to-be must appear in person at a local vital records office to obtain a marriage license. The marriage license is issued to ensure that you and your spouse-to-be are eligible to be married.

    WIC supports Connecticut Mothers and families in Breastfeeding their children; the key to good health of mothers and babies, also has benefits for families, employers and the community in general. Find information on developing a plan to work and breastfeed and access to WIC nutritionists and peer counseling services.

  • Child Advocacy

    The Office of the Child Advocate assigns a Child Advocate to a child. The Child Advocate has the following rights and powers to: initiate or intervene in court cases on behalf of children, obtain access to any records necessary, subpoena witnesses, records and documents needed to carry out an investigation or review and communicate privately with a child in need of services of the Child Advocate.

  • Aging and Disability Resources Center

    Aging and Disability Resources Centers provide consumers of any age and ability with benefits counseling, person-centered planning to help identify available long and short term services and supports, application assistance and general information and assistance.

  • Elderly Nutrition Program

    The purpose of the Elderly Nutrition Program is to provide meals, reduce hunger and food insecurity, promote socialization and the health and well-being of older adults. The program provides nutritionally balanced meals to eligible residents 60 years of age and over and their spouses. Meals are served through community cafes or delivered to the home-bound older adults.

  • The Congregate Housing Services Program (CHSP)

    The Congregate Housing Services Program (CHSP) provides supportive services for participants in eligible housing projects who meet specific requirements. The supportive services may be provided to frail, elderly persons at least 62 years of age or non-elderly persons with disabilities for the purpose of promoting independence, preventing unnecessary institutionalization and helping people remain in the community as long as possible.

  • The CHOICES Program

    Connecticut's program for Health insurance assistance, Outreach, Information and referral, Counseling, Eligibility Screening. This program provides information to persons age 60 and older as well as to people with disabilities, and is comprised of both staff, in-kind professionals from local service agencies and volunteers.
